Look at all those analgesic studies that have been done. It's clear you don't have the variability in the "same" subjects, but, in a well designed study with sufficient power the estimate of variability in placebo effect is sufficiently accurate for the purpose. In many cases, there is no other way to do it. Obviously, the study design is dependent on the indication, severity of disease state and practicality/ethicality (you can't pull teeth from a subject without medication one week and with medication the next).
