2 cmax after a single SC dose

Hello NONMEM Users, NONMEM outputs for few subjects look strange. Some subjects have 2 Cmax (IPRED) after a single SC dose. One Cmax looks normal. Another one is a smaller bump before the normal Cmax. Otherwise, the problem runs OK. Can it be an integration error? I use ADVAN13. Thank you, Pavel

Is this a simulation run? May be residual error? I never seen integration problem to result in incorrect profiles. Also check the data file, may be extra dose was given? Leonid

Hi Pavel, Since you say that IPRED shows two Cmax this could not be residual error. Like Leonid I would believe this is rather an error in setup (control stream or dataset) rather than an error with nonmem. However, since you are using an ADVAN13 model, I suppose if the dose is split into absorption into two compartments; if you have zero order absorption into a deep compartment you may see this kind of behaviour. Since you only see duplicate Cmax in some subjects, I would suggest you look at these subjects to find out how they are different. Is it dose/formulation, the observation time points, or eta values that are different from the other subjects? Best Jakob
