Try "Nonlinear Models for Repeated Measurement Data" by Davidian and Giltinan, Chapman & Hall, 1995. Excellent book overall.
There is also a lesser known book, "Linear and Nonlinear Models for the Analysis of Repeated Measurements" by Vonesh and Chinchilli, Marcel Dekker, 1997. The book comes with a SAS program called MIXNLIN.

After those books, if you want a purer source of the mathematics behind the
methods, especially behind Gallant and Davidian, I strongly recommend:
Yasuo Amemiya

He has published at least one book.
