between population-predicted values (say for concentration) and

Population predictions vs individual predictions

When you perform a population analysis, the program returns the
population means as well as the individual parameter estimates (mode of
the individual conditional distributions). If you plug the population
means values into your model and for each patient, you will get back the
population predicted values for each patient. If all patients had the
same dosing conditions as well as the same observation times, then the
population predicted profiles would be the same for each patient.
If you plug now the individual parameter estimated into your model and
for each patient, you will get the individual predictions for each
patient.
