This email is of interest only to those who install NONMEM in OS X.
I recently reinstalled XCode (version 3.2.3, obtained from the Apple developer
web site) on my Snow Leopard installation on which I have Intel Fortran (11.1)
and NONMEM 7. NONMEM ran but would not create tables. Reinstalling ifort
followed by NONMEM did not solve the problem.
I then deleted XCode 3.2.3 (in /Developer) and installed version 3.2 (which
came on the Snow Leopard installation disk), followed by reinstallation of
ifort and NONMEM. Tables were now created. Without an exhaustive test to
satisfy Koch's postulates, I cannot confirm that the culprit was XCode 3.2.3.
However, if you encounter this situation, I recommend reverting to the earlier
version of XCode.

The specific error messages that Dennis received were:
1. CONTROL 3: Did not run. The output contains the following:
0THE INPUT MATRIX IS SINGULAR.
1UNABLE TO OBTAIN A SATISFACTORY INITIAL ESTIMATE OF VARIANCE-COVARIANCE
COMPONENTS
Interestingly, I removed the $COVR entry and NONMEM ran, but no table
(same problem as below).
2. CONTROL 5: Ran. However, it did not create a table. The output
contains the following:
0THE INPUT MATRIX IS SINGULAR. SOME OF THE DIAGONAL ELEMENTS OF THE
UPPER
TRIANGULAR MATRIX U OF THE LU FACTORIZATION ARE CLOSE TO ZERO.
