I however would encourage you to make an anonymous version of your model so you can share it with the group if it does not solve the problem. It might be informative to run the table step without estimation (i.e. just with the initial values - only remove $EST and $COV) to see if PRED is still zero. If not, there may be some numerical issue propagated from EST/COV. Anyhow, the problem is intriguing. Please let us know of your progress.
