It is probably because NONMEM uses drug amount in mg internally when
calculating differential equation solutions. Thus the equation is...
dX/dt = - Vm*X/(Km + X) [or maybe even dC/dt = - Vm*X/V*(Km + X)] Km
needs to be in mg to match the units for X.
